New insurance - PLEASE HELP~!

Ok,  I just found out our new insurance (in Ohio) will be:

Cigna Open Access Plus "Bronze Package"

Any info on this insurance as far as getting banded (lapband) would be GREAT!  I cant find any info on their website except the list of hospitals they perform bariatric surgery at.

Does anybody know any info on this?
