I have molina what are there requirements?

Hi I have molina and I am just wondering has anyone had the wls and had molina and if so what did they make you do to get approved and how long was the process any help is very appreciated Thank you.

I have Monlia. I am  currently waiting on approval. My doctors office told me to allow her a week to get it sent and them recieve it, then allow 3 weeks for the approval!  I have talked to other members who also have the insuracne, they have all told me 2-3 weeks. Monlia requirements are very EASY!! Two years medical records, psy evual, pcp cleance and letter, and the test that all insurance require.  There is no required diet.  You must have one co morb. I have sleep apena!!! What doctor are you using? My doctors office has someone that just works with state insuracnes so I am pretty sure she knows her stuff!!

I went to mount carmel in columbus Dayton is kind of far they are wanting $300 for the registration and $200 for the nutrition class thing and it is a requirement to go to there nutritional class rather than somewhere else. Anyone know of anywhere in Columbus that accepts molina and you dont have out of pocket expense? Thanks for all the help too by the way.
