ok all you new yorkers i need some help!

hello eveyone, i am a former new york. boy do i miss home.. i need all your help for a friend of mine who lives in ny she is having surgery with dr.carauna probley not spelled right but oh well she is now at the point of filling out the 5 yrs diet history. does any one have an example of how the doctor wants it filled out i cant seem to get the sample up on his web site. and what does she need to put on this i didnt have to do this part as i had wls in va. she is a little scared to post on here so i am doing it for her.feel free to email me at [email protected]. and if there are any of you out there that are going though this or have already had the surgery with carauna please get in touch with me so i can put you in touch with her she is going to need all the support she can get i am hear for her but im also 500 miles away.thank you all in advace. i know my fellow new yorker will come thought for me!! love, holly 259/169/125 pre op/current/goal

Holly, Well please tell your friends welcome and that we wish her the best. I know for me that the letter had to be what the Ins Comp wanted so I got an example letter from my surgeons office. Maybe she can ask what letter she should be using as all Ins Comp require different info. I would sent you a copy of mine but I am in the middle of moving and I have no idea where all that paperwork is right now. Just a thought.... Take care ~Hugs~ Debra P
