I NEED HELP

I just got a call from my surgeons office and they need a medical history letter from my PCP before they send everything out to my insurance company. I'm trying to get this done ASAP because I really want to have surgery by the end of August. Does anyone have a sample letter I can take notes from? I don't need proof or documentation on weight loss programs I've tried, I just need to list a few that I've been on in the past years. I would like to give my PCP an idea of a successful letter... or even write it myself and just have him sign it. I am not very good at writing.... especially letters so any help would be GREATLY APPRICIATED. Thanks in advance. JT

Hi JT - Did you ask your surgeon if they have a form letter they recommend? I used the form available on my surgeon's website. I don't know if it will suit your needs, but maybe it'll give you a good jumping off point to create your own.
