Example Letter

Hello all..... I am having my family PCP act as my Endocrinologist because he has been checking my bloodwork and all for the last 4 years so he said he would be more then happy to write me an approval/consult letter, clearing me on that part, taking care of one of my pre-op consults. The questions is does anyone have an example letter that would work for my PCP to use? I have to write it out and he will just sign it but he is not sure what they want. Can anyone help me? Are you sure they will except your PCP as your endocrinologist? My PCP always ordered my blood work and I never saw an endocrinologist but i was told I had to in order to be approved...You might want to check first.

Michelle, Hello hun, I hope you are feeling great now...... I checked with Dr. Garber and he said it would be fine as long as he has a copy of my current bloodwork and a letter that clears me on that part. I am just not sure what and how the letter should read. I asked Dr. Garber and he just said to say that I am cleared for surgery. Thanks for your concern, I would hate to do ANYTHING to slow me down more since I am on a waiting pattern now. *laughing* Take care, Debra Peace
