Excellus BCBS

I have Excellus BCBS does anyone have this insurance? If so, what is the approval process like with them?

Every Employer has a slightly different policy and riders....you need to call and find out what your EXACT coverage is.  You can ask for the gastric surgery policy specifically or your plan....and make sure they don't give you their generic policy, ask if your policy covers it and get the exact rider....
