a word of caution: not all Empire policies are the same, so whatever advice you get on here may not apply to your particular policy/plan. Its aways best to call the insurace directly and ask them, they should be able to tell you their guidelines. Plenty of surgeons in NYC area accept Empire in general, but that doesn't guarantee approval (and pre-approval does not guarantee payment). I happen to have empire bc/bs EPO plan, and it didn't require a pre-op supervised diet. all i had to do was a psych consult, 1 dietician consult, and then regular medical clearance (cardio, lungs, primary doc). and they approved my sleeve on first submission. My hubby's was denied at first (he used a different surgeon but we have same insurance), but was approved on appeal.
