I HATE puking-hard time slowing down
So I had my surgery on 4/8. I am told to progress and stretch my pouch a bit. I am having a terrible time slowing down. I puke at least 2 times a day. Mostly at dinner.... I totally forget to slow untill I am in pain and have to bring it back up. I have tried to take a bite, go do something, come back for another, leave again... dinner is freezing cause it takes me like 30-45 minutes to eat it. I thought I couldnt do chicken cause it always came back up, come to find out I was just eating it to fast! UGH! Any tips on slowing down??? And I am still having a hard time getting all my fluid in. Before surgery I drank no water... only like a 12pk of pepsi daily, OMG!!! Now I get in only about 30-40 oz a day on a good day. Why cant I use a staw??? I always drank better w/a straw. Ijust cant get it in...HELP!!!
On a more positive note Im feeling GREAT. I can beath much better, I am lighter than my hubby for the first time ever, I have no more edema, My knees dont hurt, I am 3 sizes down, and I could go on and on... so I am so glad in my decision to have the surgery!!!! Thanks for all your support!
Kristy
Kristy,
I know where you're coming from about having to slow down. (I used to be able to down a big bag of McDonald's crap in record time!!) It only took 2 times for me to learn to remember to slow down. I know the food gets cold when you eat slow - I would zap it just enough to warm it up if I had to... it was better than scarfing it down & getting sick!! I made sure that I put the fork down between each bite & what worked for me in the beginning (although we are supposed to pay attention to what we eat) was to do a crossword puzzle or read a magazine article or a chapter in my favorite book that totally engrossed me so I would "forget" about the food. I don't do that anymore, the habit has become engrained so try to find something similar that might work for you for now.
About the straw - I was told that it caused too much air to get into our pouches. I still have a hard time remembering to take straws out of drinks at restaurants & I admit I have slipped & had a few sips out of one before realizing what I was doing. I didn't have any negative reactions but I didn't take more than 3-4 sips tops!! I would talk to Evelyn or someone at Dr Clarke's office & tell them that you are having a hard time getting in your water - it is so important to stay hydrated!! I take full advantage of those few hours in between meals to drink, drink, drink! I always had a big glass full of ice & water close at hand & I even put a posted note on my monitor at work to remind me!! I was a big water drinker before surgery so it wasn't a hard switch for me but a coworker had the same problem you are & what worked for her was to buy a few bottles of water in the smallest size she could find & she would just keep filling them back up! She kept track of how many bottles she drank & it dodn't feel overwhelming to her because it was a small bottle sitting in front of her rather than a large one.
I am sure everyone else will have some great tips too!! Don't give up - keep trying! You will find something that works for you & before you know it it will become habit & you won't even realize you're doing it!! You are doing great - keep it up!!

Fluids is tough. Just remember to sip sip sip all day long. Dont go anywhere without some sort of drink. As for eating, my suggestion would be to sit down and take a small bite and chew it like u never chewed it before and then wait till the clock goes by another minute and try it again. Make sure that you are chewing as much as possible. Slowing down is hard believe me. I am confident to say that pre op I could eat anyone here under the table. SLOOOOOWWWWW is the best way to go.
Oh..I can soooo relate. I am a "wolfer" from way back. I find that if I cut up my food immediately..then eat...that helps some. I still have problems with pastas and bread. I guess that might be permanent...but, it is so worth it all. I am down 116 pounds with 11 to go and I feel like a million. Good luck...chew...chew..chew!

Hello,
Vomiting 2x day is not typical or expected. Are you sure it is purely behavioral or could this possibly be a mechanical issue? (Stricture) talk to the surgeon if concerned it may be mechanical..strictures are common and can increase in tigtness over time so if you find slowly u can not even drink or eat things u could before or pills come up!!!CHECK IT OUT!. What do you think the fluid thing is, behavioral or mechanical (hard to drink, painful etc). Can you keep pills down?
Different folks progress with pouch healing at different paces and an ANGRY pouch stays that way for hours to days. Some go back to liquids for a meal up to a day or 2 when they vomit!!! Eating over 30-45 min may be too long and can backfire longterm (you will be able to eat more calories this way). Is it possible to set a timer and every minute take a bite? chew for a minute, practice until it becomes more habitual? Go back to more pureed type of foods especially at night (like chicken salad, egg salad, tuna salad all mixed in a magic bullet and or mini chopper like pate) Sometimes meat works better in am than eve (someis stress related we are tighter at night if more stressed!) some do meat in am and more breakfasty stuff in eve....for a while at least.
Foods slow cooked such as crock pot, chicken thighs seem mositer than breasts, add some lo carb BBQ sauce or regular and remove sauce before eating..real tastey! Same with pork this way works well for many folks...
Straws are not recommended as many bring in too much air with them BUT some actually find they work better than GULPING air, I guess trial and error, it isn't that straws are totaly a NO NO, test it out!
Here are some other thoughts use a 3 min egg timer for food, take bite and chew and have next bite just when flipping it over. Eating dinner that is cold like deli meats roled up or the tuna salady type of things may lessen the hot issue for a bit!
Are you doing orotein shakes at all? How much protein are you getting int (I am concerned for your health and hair! LOL) esp. if you are vomiting. As others suggested cutting food up to tiny specs before you eat or grinding may help at this stage, it is a passing stage!

Focus on Fluids (*I have posted prior may help, esp read tips at bottom!)
One of the main challenges for a gastric bypass patient over time is to maintain hydration. Your body needs just as much fluid after the surgery as it did before surgery, yet the new stomach pouch limits you to 1-2 ounces at a time. (NO MORE GUZZLING!) Maintenance of fluid intake may also be difficult because of the necessity to take in food or fluids, and not both at once. You must compensate for this by taking small amounts of fluid on a near-constant basis. Usually this means that you should carry your fluid source with you at all times.
*** We cannot overemphasize that the liquid must be a caffeine, carbonation & calorie-free. Patients who use fruit juice or other calorie-containing liquids for hydration may experience inadequate weight loss because of the calories in these fluids, and can experience dumping syndrome from the sugar. Caffeine is a possible appetite-stimulant, diuretic and irritant and can potentially cause ulcers in the new pouch and add to dehydration and low-iron blood (which we already poorly absorb iron post-op). Carbonation can possibly cause gastric irritation, if carbonated beverage also contains phosphorus it may act to leach calcium out of bones, and some feel it may stretch pouch/stoma.
Drinking at least 64 ounces of fluids post-op is a necessity. Water is the body's best source, and serves the purpose to aid digestion and absorption of food; it regulates temperature and circulation, carries nutrients and O2 to the cells of the body and removes toxins and waste.
Dehydration can cause/aggravate constipation, kidney stones, urinary infections, high or low blood pressure, difficulty controlling diabetes and migraines. Dizziness, clumsiness, muscle cramps in arms and legs, dark yellow urine, fatigue, irritability, are sunken eyes, low blood pressure and fainting, bloating, and a fast, weak pulse -- are signs that dehydration is setting in.
Fluids should be consumed on a near-constant basis, but not during meals and not soon following food. If you drink during your meal, you will tend to wash the food out of the stomach pouch, and consume more food than is desirable. It may seem strange at first to eat without drinking but it works fine. A good tip here is to actually not have any fluid on the table while you are eating. In addition to reducing your overall food intake, this new habit will encourage you to chew more thoroughly before swallowing, because you will not count on the fluid to wash down half-chewed food. Also note that soup is kind of like drinking liquids with food - soup is not an absolute "no-no" but recognize that it should not be a regular part of your diet.
Liquids should not be consumed for about 30-60 minutes after eating. This is because the food actually forms a plug that blocks the outlet from the pouch until it has all passed through. If you drink liquids on top of this plug it will create a very uncomfortable sensation and may also force food downstream more quickly than it should go.
A brief review on coordinating liquids and food:
1. Sip liquids constantly, even just before eating. Don't drink liquids while eating.
2. Don't drink liquids until about one hour after completing your meal.
3. When the stomach pouch is mature (six months or more after surgery) it is useful to wait two or three hours after eating before consuming liquid.
If you must choose between liquids and food (frequently true in the first months after gastric bypass) - choose liquids!
CLEAR LIQUIDS IDEAS:
***(VARIETY IS THE KEY TO GETTING THROUGH THE FIRST 2 WEEKS!)***
1. Broth any flavor (canned or made from bullion cubes) or strained soups such as chicken noodle without the noodles! Cup of soup brands strained.
2. ¼-½ strength apple or cranberry or grape juice (OJ may be too acidic at first).
3. Full or ½ strength Ocean Spray lite cranberry juice varieties (white, cran-grape, cran-raspberry, ****tail) these have 40 cal a glass vs 120 cal a glass of regular juices and are made with splenda. Available at Wal-Mart and PC. They also have lower cal 10 per glass also!
4. Diet V8 Splash (Wal-Mart and PC) 10 calories a glass and made with splenda, full strength or watered in ½.
5. Carb Countdown dairy beverage. *has lactose.
6. Old Orchard Low carb juices (30-40 calories a serving with 6-10 G carbs/with different varieties) full or ½ strength. Cranberry, Cranberry Raspberry, Cranberry Grape, and White Cranberry flavors available.
7. Diet Very Fine juices.
8. Crystal lite (Wal-Mart) any variety (decaf tea, lemonade, fruit punch etc), diet Tang (Wal-Mart) diet Kool-Aid any variety (Wal-Mart). OR Carb options & Wyler's Light powdered beverages. NEW Wal-Mart-brand called great value, 4-C's and Crystal lite have to go sizes! To add to a 16 oz bottle water! NEAT!**NOTE MANY PEOPLE WATER DOWN EVEN MOR FINDING IT TOO SWEET***
9. Decaf tea or herbal decaf teas or decaf coffee hot or iced.
10. WATER, try it iced, warm or with a little lemon or lime to flavor, many like to add a little splenda to change the taste! Water may have a metallic taste initially this is normal and goes away.
11. There are many flavored waters available like propel called poweraide (diet variety only)
12. Propel Fitness water by Gatorade: Kiwi Strawberry, Peach, Orange, Berry, Black Cherry, and Lemon.
13. Diet Jell-O (you make or premade varieties). DRINK IT WARM! YUMMY!
14. Sugar-free Popsicles (Popsicle brand has original and tropical fruit flavors.
15. NO-Sugar added Fudgesicles (popsicle brand).
16. New Minute Maid light (Light Lemonade, Light Guava Citrus, Light Raspberry Passion & light mango tropical) It is a 5 cal/serving
juice (2 servings in a bottle)...0 fat 80 mg sodium 2 g carbs (less than 1 g sugar) 0 protein...(It comes in 4 packs 16.9 oz plastic bottles or 12 pk 12 oz cans).
17. Wal-mart has SAM's brand (called First-Harvest) non-calorie non-carbonated flavored beverages: cherry-limeade, lemonade, strawberry Kiwi and MY FAV peach-grapefruit. (4 Try true lemon, lime or orange at www.truelemon.com to flavor water!.
***TIP
Keep a journal of each hour in a day for the first week of fluids. Write down what you drink in the hour and what activity you do! You should move every 1-2 hrs even if it is just walking around the house to avoid respiratory or circulation problems. This helps keep you mindful! Remember you cannot catch up on fluids later in the day OR take a 2 L soda bottle empty and fill it each time you sip some fluid with same amount this is a great visual as to how far to go and how much u have drank, set midday to have it 1/2 gone!!!!

I never heard this before about waiting 2 to 3 hours after eating to begining drinking again, 6 mo after surgery.... Is it something new or did I miss this all together? Also I was told to stop drinking at least 20 to 30 before eating.... Am I reading this wrong???

It is not new at all, this is directly from POUCH RULES http://www.waynesmith.net/lapband/faq/pouch_management.htm (an except from an article: Flanagan, L. Measurement of Functional Pouch Volume Following the Gastric Bypass Procedure. Ob Surg 1996; 6:38-43)
Many say no fluid for 20-30 min prior to eating, after the first 2 mo or so anyways, it flows through and doesnt fillyou up...I have done this all my WLS postop life w/o issues..I agree when I wait 2 hrs after eaing to drink I stay full more!

Thanks for all the awesome ideas. You are all the BEST!!! I think it is me...not a stricture. My mom had a stricture and couldnt keep anythng down. I only bring up the fluid if i drink it to soon after i eat. I can feel it just sitting in my chest. I dont really VOMIT... not like I have in the past when I had a virus... its more like bringing it back up untill the pain in my chest goes away. Yesterday it was only at dinner. I had meat...but I was on dinner break, working in the ER with my teacher, and they were so busy...I know I ate way to fast. Lunch I had panera broccoli soup and that was fine... I will try all those fluid ideas. I dont use protein shakes cause I HATE every one I have tried. I do have the Pure Protein bars with 19g of protein and Atkins Advantage morning bars... really good. I wish there were a good protein drink out there that I could stomache. I know for sure I dont get in 60 g of protein a day...not w/the little that I eat, I worry too asbout that. I do eat those bars, 1 a day... and a lot of cheese. I go back to the doc, well PA, in August, if it gets worse I will call them. Thanks again for all the ideas.
Kristy
Yes indeed slowing down and taking care of ourselves is one of our challenges to learn, especially in the midst of business/chaos....One trip to the Er for dehydration has been a great learning opportunity/lesson for many folks to somehow get in their fluids. Soup as you can see is a slider and toelrated easy, just like a liquid it flows through the pouch w/o issue typically...An ok choice once and a while, the further out we are the less attractive this seems as meal choice as we consume quite a few calories w/o any fullness. Ok so you feel it is behavioral, hoping you figure out the water and protein thing before August!!!
Yes many do not like protein, some figure a way to get it in (frame it as a life saving medicine that tastes bad! LOL) and wanting to avoid the muscle wasting, slowed metabolic rate, decreased healing and fatigue that can accompany it (and of course hair loss) many say they find adding unflavored protein to food stuff helpful. There is a belief that initially the protein in food/bars is less absorbable due to our altered guts, where the predigested whey is more absorbable..? Protein Bars... The Good, The Bad, And The Ugly!
http://www.ultimatefatburner.com/backissues/0252002.html
Some folkslike protein warm, in decaf coffee they use the chocolate like a mocca flavor using coffee pot temperature stuff. GNC also has bottled (expensive) isopute that is like koolaid many ike 40 g in bottle!?
I know the protein bars are better than nothing and a good emergency back up, I seemingkly had to let them go as they were trigger foods, wolfe in sheeps clothing (glorified candy bars)..I used them a lot early postop for 2 yrs or more then as with the SF products I let them go....Wondering if the cheese and lack fluids causing constipation? I hope not but I know I had that severe!! ;-(
MOST people refer to these when using unflavored protein: (although I have hear of beneprotein??)
ANYWHEY is an unflavored protein. You can cook with it too!
(***70 calories a scoop/0g fat/1g carb (no sugar carbs)/17 G protein)

UNJURY (UNFLAVORED) Ingredients: Pure Whey Protein Isolate
UU is good to add to cold and warm beverages and soups, but not HOT liquids.
Do not heat the beverage or soup above 130 degrees F.
UU works in baked and cooked recipes such as brownies and scrambled eggs.
IMPORTANT NOTE: high cooking or baking heat does NOT destroy the protein or the protein value, but in liquids, the heat can make the proteins more clumpy. Even if this happens, you are still getting your protein. It's much like cooking eggs or meat. They change texture when cooked, but cooked, they still provide complete protein.
In many recipes you'll find UNFLAVORED to be virtually undetectable, but some will notice some flavor in some recipes. For example, in Crystal Lite, some detect no flavor to UU, and some can tell it's there. As with all UNJURY products, we guarantee 100% satisfaction.
Thank you very much to all of you who have tried it and given us your great suggestions and recipes!! We will soon have recipes on our Recipes page, but here are some initial ideas for ways to use "UU":
· Jello INSTANT sugar free pudding
· Crystal Lite and Sugar-Free Tang
· Chicken broth (not HOT, only warm)
· Soups such as cream of chicken (not HOT, only warm)
· Scrambled eggs
· Mixed with milk it is undetectable if you just want to have the flavor of milk...on cereal or in a glass.

· Orange Juice
