I was just approved by HIP/ Medcaid

I use to be with United Health Care/ medcaid but they need 6 months diet notes, preop testing ,and medical necessity from pcp. That means I would had a date sometime in feb. 2007 HIP requires pre-op and medical necessity letter. I change to HIP 09/13/2006 but effective date 10/1/2006 then my paper was put in my surgery date 10/24/2006. I hope this was helpful to any who do not have diet notes or don't want to waste any any more time. GOD bless

Good for you... I have UHC (group plan from work) and did not need a 6 month diet history. It was on th orgional check list but the surgical coordinator said I didn't need it. UHC wanted a 5 year weight history which my PCP was just able to say that I have been to heavy to weigh on her office scale for the last five years. But my BMI is 52 and I have severe sleep apnia so that might have played in to it as I got approved with the first letter.
