Nutritional Evaluation question

Hey I have a question when you go to get a Nutritional evaluation what exactly is the insurance company looking for?  I mean what does they look for the nutritionist to say in her evaluation?  Does anyone have a copy of their nutritional evaluation?  The reason I asked is mainly the one I was going to she know how to give a nutritional consultation but she doesnt know if there is something specific to my surgery that she needs to add.  So, let me know what it should consist of.

My insurance company did not require a nutritional evaluation.  My surgeon did require that I meet with a dietician, but it was more about her telling me what I needed to eat and not so much about her evaluating me.  I recommend you call your insurance company and find out exactly what info they need.
