Obesity home nurse help

I am interested in pursuing a career in nursing and nutrition. I was wondering what the possibility of starting company that gave home care to the obese would be. Diet and workout plans would be provided and if allowed, I would stay at their home monitoring the workout and diet.  I've never heard of a career that did that and I'm not sure if I would be able to have that much freedom.
