HMR, anyone done this successfully?

Hi, I'm new to this forum... I had a bypass in 02 (RNY) lost over 100lbs... maintained most of it. I still have at least another 100 to go, my surgeon doesn not want to do a revision and has recommended that I try HMR under his supervision for 4 mos... anyone have any experience with this? Thanks, Lisa

It is a hospital supervised liquid protein diet. They generally use it for folks who have to lose 100 lbs before they can be considered for a gastric bypass..

Okay, my opinion.  HELL NO!  DON'T DO IT.  Okay, I know that's harsh and I state up front this is my opinion.  Take from it what you wish, but you asked for thoughts.  I also, for the record, rarely support the concept of WLS.  I understand however, that you're thinking to do this to do WLS.  So, my post may be unwelcome by you.

A liquid protein diet is a VLCD, very low calorie diet.  Brazil, I have learned a lot on my recent journey with weight loss after being overweight all my life and on countless diets.  My heighest weight was 340, I started this journey at 320 and have lost 50lbs.  My goal weight is between 180 and 220 because of my bodytype and frame.   From what I've learned, a VLCD will do this--it will likely make you lose the 100 lbs (though someone like me, I couldn't lose on Medifast, so that does happen).  The problem is, I guarantee you will regain it the minute you return to food and go over the amount of calories you were on while on the VLCD.  This is what Oprah went through, remember?  See, this is the nasty secret of commercial diets (JC, NS, LAWL, WW) and WLS (some here don't agree with me and they don't have to, I stand strong on information).  If you eat below your BMR for long periods of time, you treat your body to come to think that this is the amount of calories you are going to give it.  Now, your BMR is the amount of calories your body needs to just function (I have a link on my blog to help you figure out that number).  If you have yo-yo'd in the past, you've likely done this before, eaten too little to lose and primed it to regain after you return to any calories above that diet amount.  Each time you go below your BMR for long periods of time, you further ruin your metabolism.  However, that can be fixed, but that's another topic. The upshot is that you will have to remain on VLCD to keep the weight off.  That is up to you if that is something you are willing to do.  For me, it wasn't.  I instead chose to lose weight with activity and eating over my BMR, letting activity burn the calories for me, so that when I get to goal I will be able to eat a normal amount of food and be satisfied with it and maintain the weight loss I have worked so hard for. I say this not to sway your choice, you have the power of your own decision, but because I want you to make an informed choice.  I'd much rather see you eat at BMR and low carb and do exercise than see you prepare your body for unsustainable weight loss.... Jersey, thanks for the input... I have many, many questions before I make any decision on this and you bring up some very important points that I will be discussing with the dr. on Friday... I appreciate your honesty.  Sounds like you have your head on straight too! Good job...

More opinion?  Docs aren't the ones to necessarily trust re: weight loss.  They get less than 5 hours education on it and often pay attention to the media's or cultural perspective on it.  Also, not all are well educated about BMR.  Please, if you feel the doc's input is one to trust ((shrug)), go to the research section on my blog and print what you think is relevant.  And ask him/her to respond to the scientific studies not just share their opinion. Oops, my bad, you ahd teh WLS, if it was RNY, what's the point of a liquid VLCD?  Did the doc say?

That I would be successful doing this short term, then transitioning back to the beginning rules of the bypass accompanied by exercise. He feels that I am taking a huge risk if I get a revision by him or anyone else because of the bleeding issues I had during the original bypass surgery.  I see the Nurse Practitioner to discuss all the details about the HMR diet... I have loads of ?s to ask and I really appreciate everyone's opinions. Thanks

VLCD, HMR, liquid whatever... None of that is going to work long term.  Are you still eating like an RNYer or are you able to eat larger portions now? What about carb, sugar and carbonation intake? Before doing a liquid diet I would look at your current eating habits and see where you might be able to make some changes. Perhaps you can make some small changes to start the weight moving downward again?  Up your exercise, lower your carbs, etc.. Otherwise I would recommend going to see a true DS surgeon who is comfortable with revisions. Visit the DS or the Revisions board.  You'll be able to find info on great revision surgeons. Chances are your surgeon may not be comfortable doing a revision and is steering you away from it because of that.

Chris you make a very good point about the surgeons comfort level... I thought of that too and I also have an appt. in Boston with a bariatric surgeon that does  a lot of revisions... I am able to eat more than I did 6 yrs ago but in comparison to others I eat less than 1/2 of what they do. I definitely have fallen off the wagon as far as doing everything right... time to get back to the basics... I just want to finish what I started in 2002.
