Does anyone knows what a pre-determination form is?My insurance company sent my doctor one today. Thanks

Hi, Dr. Bilof was my surgeon. Dr. Nussbaums partner. Great Doc's!! I don't know what that form is, you can call your insurance and ask them or just call the surgeon's office, they will tell you. Tracy

You have the best surgeon in the USA. He is mine and I had my surgery in December 2005.

I love Bergen Bariatric in Englewood Hospital. Problem is they are not in any network except Oxford and maybe not even them anymore. They are excellent. They have a great support group. I think they do payment plans. They are in it for more then they money. Very caring doctors. Even though out of network they still have to get pre approved.
