I have Cigna's Coverage Position on WLS/Requirements

Hi Mary, I emailed you a copy but will probably have to send it from work because I don't think I havce the right format here at home. I am scheduled for surgery end of Decemebr, unless Cigna approves me then I will be having my surgery inNovember. Fingers crossed. Susan
