DO ANYONE KNOW WHICH PRETEST Are REQUIRED FOR approval? i know they require a psych eval. Are there any others?

Dawn, The best thing to do is call the insurance company and ask them what is required, This surgery is alway changing so the requirements are changing also I have found.
