Help............Need example of letter from nutitionist

Hello All,
If there is anyone out there that can send me a example/guide line of the nutritionist letter needed by bc/bs PLEASE send it to me. I went to my doctor for 7 months to check my weight and discuss my eating habits, but she didn't write notes other then my weight. I only have two more weeks to get my request for lap band surgery sent in or I will need to start over again...I just can't take that.

your nut. should know what to do.....if not they should call your pcp or surgeon to ask for a template.
i believe you must meet with a nut as part of the requirements as well, just discussing it with your PCP wouldn't count anyway.
i have BC/BS and had to meet with the shrink, the nut and my pcp, who all sent the appropriate letters to my surgeon's office and l was approved in less than 24 hours.

The problem is I went to my nut for 8 month then I was told by the wls center because it was longer then one year I needed to get my weight noted again for 7 months so my primary doctor check my weight (no charge) for 7 months (Now I know why they looked at me like I was crazy when I would bring in a list of my meals) anyway they only noted my weight in computer. It should be 7 months of notes & my weight. The weight loss center never told me prior to my last visits with them 6 month ago that bcbs changed there rules. When I started journey I needed notes within two years.

OK then; what you need to do is contact your surgeon's office, speak to the insurance coordinator and have that person tell you, IN DETAIL, exactly what they need for your specific insurance.
If your PCP just noted your weight, that is up to your insurance company to decide. I don't recall what my PCP sent, but I do remember that he sent a letter stating why he recommended me for WLS (I did this whole process two years ago, so I really don't remember.)

2 of the things that BCBS requires is a letter from a nutritionist or some type of nutrition professional stating that you have been educated and understand the nutritional facts and what things you will need to eat and avoid after WLS, and also, a note or notes from a Dr. or professional with your weight and height (and sometimes BP) for 6 months (I believe it's consecutive months). As long as it has a Dr. or professionals "stamp" or letterhead then it's valid. And of course, their requirement is that your BMI = 40 or a little bit lower with co-morbitities.
My insurance didnt' require the 6 month diet with the doctor but when I went to the first surgeon, he did require it. So, I did it anyway and when I went to the final surgeon that I used, it wasn't needed. But this is what is needed. It must be by a doctor - not nutritionist. They must meet with you for 6 months consecutively and keep tract of your weight on these 6 visits. They must also go over what you are eating and go over what is suggested to help you continue to lose weight. This must all be documented. Mine also documented any supplements I was taking to help me with my preop preparation. My NUT gave me sheets to fill out where I listed everything I ate and we went over it at each appointment. This way it was in my file and could be used for the approval process.
