I had Gastric Bypass in Sept of 2003. I am back up to 350#. I have had two abdominal surgeries after my bypass by my surgeon Nicholas Bertha. Now I can't get a call back from the doctor in his office that he referred me to. Anyone experience similar? Last nite I called Bertha and told him my primary wants me in the emergency room. He advised me aganist going to ER saying they won't know what to do. I am at a roadblock without guidance from his office. See letter below I sent them.

Dear Dr. Vincent Iannace MD

Thank you for taking the time to see me this afternoon. I cannot reiterate enough to you the increasing groin pain and discomfort that I am in.   I initially saw Dr. Mariana Grundman, MD at my primary doctor’s office, Avenel Iselin Medical Group. She said that she felt a small hernia and referred me to a surgeon. I called your office at that point but was told your doctors were moving the following week and it would be two weeks before an appointment.  I then went to Dr. Wein, MD, a urologist, in Edison who told me it was a muscle sprain. (you have the report). The pain got progressively worse so I made an appointment with Dr. Rajesh Rao, MD., a pain specialist in Morristown, who again diagnosed it as not muscular or skeletal but a hernia and referred me to a surgeon. I have faxed his office for release of his records to you. I then went to a scheduled follow up visit with Dr. Edward M. Decter my orthopedic surgeon in West Orange NJ. I told him of the groin pain. He examined me for this and took x-rays to rule out back or hip related pain. He also diagnosed a hernia and referred me to a surgeon. I have also faxed his office for release of those records to you.

The pain getting more frequent, I went to see Dr. Yvonne Nelson  MD, also at Avenel Iselin Medical group. She is my actual primary doctor of choice.  To rule out a hernia, she scheduled a Cat Scan and Ultra Sound. After reviewing the results of these she said that it was definitely a hernia.

I then went to Dr. Nicolas Bertha in your office and was examined by him. He said that the hernia was not in the usual place, but it was a hernia, and asked that I get the surgical records from a previous abdominalplasty  and referred me to you. I have attained these records, plus I got copies of the cat scan and ultra sound on CD.

I am asking that all the records of all the consults that I have recently had by these multitude of specialist be released to you, just in case they may be of help. Attached are the releases for these records to you. I have already faxed them to the appropriate doctors.

When the pain hits, it feels like I have been kicked in the groin, it is an excruciating pain. It hits often.

Thank you for your attention to my issues, I am very grateful for any help you can give.

Got a call from Dr. Iannace, he said I need a surgeon specialized in reconstructing the abdominal wall, so far all referrals I am getting from Advanced Laproscopic Surgeons lead to a bad ending, they do not accept my insurance, Horizon NJ Direct. So back to square one, in pain, and no where to go for help.

So sorry you are suffering.. Have you attempted to call the insurance company for the name of surgeons in the plan..
Good Luck and speedy recovery.
Maybe you will need to go out of network? sorry.

 Hi,
Thanks for your reply. I have a list of in network surgeons. The problem is that Bertha and Innacci are both in network for me, that and that Bertha has done three prioir surgeries on me I went back to them. They say not all surgeons will be able to handle my issue. I even suggested to them going into the ER at Morristown Hospital and finding an appropriate surgeon there. I am told by them that that is not a good option due to the complexity of my issue. I just don't know where to turn.

I hate to say it but sometimes you have to do what you think is right.. god forbid they were in this situation.. would they be waiting.. Thankfull and with God's grace I have never been in a situation like this but I would have to say.. go to the ER...
It may not be what those DR's want you to do.. but what is your gut telling you.. are you able to wait for them to have time or decide when they want to help you.. Pain is a sign the body sends that something is not right.. Listen to your gut and your heart and make the best decision for you..
Those are surgeons that work in Morristown.. if you go to Morristown.. and you tell the triage nurse of them they should be paged to my understanding that you are there and need help...
Best of luck.. I am praying for your recovery.

Its now Saturday, March 7, 2009. I spoke to Dr. Iannacci at Advanced Laproscopic Surgeons and was told he would call me back after reviewing my cat scan. I am still waiting. Received referrals by him to other doctors, none in my insurance plan. Went to see a general surgeon, reffred by my Orthopedic doc, who says that I have a hernia. He said I need a laproscopic hernai surgeon. (thought that was what my gastric bypass surgeon was). Now I have an appointment with another laproscopic doc next wee. Till then on two pain patches, and still in pain.
I even went to a neurologist for evaluation. More tests scheduled. Just trying to find a solution to my issues.

Hi, just finished reading your posts, wow, you must be miserable.  Thought hip problem when I first started reading but that's been covered.  My RNY was done by Ajay Goyal, he has an office in Union.  908-378-1779.  Very skilled, talented and experienced laparoscpoic surgeon.  My gastroenterologist is Amber Khan in Millburn, 908-522-1313.  These docs have done procedures together, wonder what they could do for you.  Do they take your insurance.  I guess my first call would be to Goyal, were it me.   In the meantime, I'm keeping you in my prayers.
