BCBS of NJ Horizon with 60 BMI

Yup, you're correct. I had my surgery in 2004 and they approved very quickly as well but things are different today. BC/BS policy specifically states no approvals with a BMI over 60.  Kathy

I too have BC/BS and I was approved thought them.   It took me 9 months to do everything that was asked (6 month supervised diet, tests,  afters tests  after tests ,etc, etc....) but once the paper work was submitted it took 13 days to hear  back from them.    My PCP  advised me with BC/BS you must do EVERYTHING they ask you to do or they will denied you.
