Help Please

Does anyone have a copy of their letter from the Psychiatrist for the insurance company? I work at a mental health agency and my boss is having her friend (conflict of interest if she did it) as a favor do my evaluation for free. I've talked to her already and she will clear me but wants me to write the letter out answering all the standard questions then email mail it and she will add/change what she needs since she is doing it as a favor. Of course I said I will. I just have no idea what kind of form letter it should be. if someone can help me I'd really appreciate it. Black out anything personal I promise it's just to get the form/feel of it. Thanks Dawn

Hi Dawn, Is this really a risk worth taking.  I'm wondering how you will feel about it later.  Truthfully, the psychologist or psychiatrist ought to be able to do this without your help. FYI , most insurances will pay for at least part of the psych eval. Dr. Mike

Thank you for the concern Dr. I feel that the benifits out weighs the risks at this point in my life. I know that if I do not do something drastic at this point that I very well may not be here much longer. I will be happy if I achive any results from the surgery I'm hoping some of my medical issues are resolved I'm hoping I can stand longer periods of time walk stairs without stopping for brakes or feeling exhusted from 14 steps (to the 2 floor office daily). I will take what I can get because at this point I have failed at all other modes of achiving weight loss. I am not unrealistic nor am I unafraid I have thought for years about this, now is the right time. As for the letter I am just filling out the basic information/answering question she will go over it and make nessesary changes before submitting it. It's just helping time wise. We have already spoke for 45 minutes. Anyway she is very nice and has offered to speak to me again if I feel I need it and follow me during and after the procedure.  Thank You

Hi Dawn, I'm afraid you missed my point. I was referring to the risk of not having an independent evaluation done, not the risk of surgery! I am totally supportive of WLS. Sorry Dr. She most definately is independant I had never met her. I work with mental health clinicans on a PACT team my boss and many others on the team have licenses but that would be a conflict of interest for them to do the evaluation. My boss has a sister in law who is also a licensed professtional she was nice enough to do this for free for me. Thanks again sorry for the confusion.

from what i understand, they basically want to know that you are mentally prepared for the surgery and what you need to do afterward. that you are aware of the risks and benefits. that you are not undertaking this as a lark because its the "in" surgery these days. i think its stuff like that. p.s. maybe your surgeon could give you a hand with this.  unfortunately that was one letter i never got a copy of. good luck, jacki

I never got a copy of my letter either.  Pretty much they seemed more concerned with any past or present addictions to drugs/alcohol.  At the end I got a "How do you feel about having to take a multi-vitamin every day for the rest of your life."  My only response was "I've been taking HBP pills every day for the last 15 years.  I'd be more then happy to swap them out."  And that was pretty much the end of it.  I wasn't impressed with the consult but then again I know what I'm getting into.  Good luck with it.

I wrote the letter myself if anyone is in my position I'd email a copy if you want, it hits on every subject the Dr.'s office suggested the insurance company would want.  Took me awhile but I think it's good so does my boss. Dawn
