Vicki, Dr. Bertha from Florham Park, NJ did my surgery. The surgery was performed at Morristown Memorial Hosp. My Dr. submitted to Ins. at 3pm on a Thursday and by 10am the next morning, I had my approval. Give it a try.  Best of Luck to you! Amy - Lap RNY 6/11/07 Surg/Now/Goal 254/219/135 84 lbs to goal

Vicki, My Surgeon's office submitted all of my paperwork for me. They sent all of the paperwork from their office, my Nut eval my Psych eval & the letter from my PCP. DEAR VICKI     I AGREE WITH KATIE'S POST UNDER YOURS.   IF YOUR PCP DOES NOT WRITE YOU A LETTER TELLING THE INS. HOW IMPORTANT THIS IS TO YOUR HEALTH THAN YOU NEED A NEW PCP.   ANY DOCTOR WORTH HIS/HER WEIGHT IN SALT CAN SEE YOU ARE TOO YOUNG TO HAVE THESE  MEDICAL PROBLEMS.   WHEN I TRIED TO GET APPROVAL, I GOT LETTERS FROM ORTHA-PEDIC SURGEON,  ALLERGY DOC.  MY GI DOC.   EVERYONE OF THEM WROTE LETTERS THAT THE SURGEON SENT IN.    YOU GO TO YOUR PCP, AND TELL HIM/HER THAT YOU NEED THAT LETTER ASAP.   LET THE SURGEON KNOW THAT YOU ARE GETTING IT, AND HAVE THEM FAX IT TO THE SURGEON, LET THE SURGEON PRESENT IT,  THEY KNOW WHAT INS CO. ARE LOOKING FOR.    BEST OF LUCK TO YOU.  KEEP US POSTED. P.S.  EVEN IF YOU CHANGED PCP..........I'M TELLING YOU,  THEY WOULD WRITE THAT LETTER RIGHT AWAY, JUST BY LOOKING OVER YOUR CHART. I havent been seeing my PCP very long. I actually just changed about 2 months ago. I always used Health clinics...less chance of seeing same doctor and being told how I was getting fat.  I will definatly see if a letter from my current PCP will help mty cause.

even tho you used a health clinic they only have 1 chart on you --get the surgeon's office to get a copy of that -- it has your weight listed from visit to visit -- it doesn't have to have appointments for being overweight as long as your weight is listed every visit -- it  can't hurt to get the surgeons office to request it  good luck roberta

I sent in all the dates and weights they had,   2001, 2005, 2006, 2007.     The 2001 weight they say they arent sure if they can use it b/c it is from the end of my pregnancy.

Hi Vicki, I wanted to let you know that I have Horizon BCBS NJ HMO, my paperwork was submitted on Monday I received and answer at 8:30 this morning (48 hours - wow)...  I also only had a few weights, so don't worry and please let me know how you make out, Good luck... Jackie

wow. that is fast.   I am frustrated that mine is so complacated.  Wonder if HMO and PPO are that different?!? Hope not.

You can get records from any doctor you have seen even your gyno can give records of your weight. Have you been a member of a gym that would help. Also just send it in you never know they might just approve it. If not be sure you rebute it over and over. They have to approve it sooner or later. I am wishing you all the best. My prayers are with you.
