Help...I need medical leave papers filled out and I don't know what to include

I'm going to Dr.Joya in April 10th and sent Gerald my leave papers but he asked me fill them out myself and he will have them signed. What can I use as Medical reason for leave so that my business stays my own. I need the most technical phrasiology you guys can come up with. PLEASE HELP. Also, Are there things I should take with me such as my own food or anything. I need TIPS from those of you who have "BEEN THERE DONE THAT" THANKS, JEWELL

All you need to put is abdominal surgery! I wanted to keep my surgery private from my company's records, so that's what we put on my paperwork. As long as it's signed by a physician, there's nothing your company can do, even if it says nosejob! Keep all the info very bland!
