JCI is "Joint Commission International". It is the International equivalent to the same organization that accredis U.S. hospitals. I have read where it is important that the International facility that you are being treated in be JCI accredited. I have attached some detailed information about JCI for everyone's information.
About Joint Commission International As the International division of Joint Commission Resources, Joint Commission International (JCI) has been working with health care organizations, ministries of health, and global organizations in over 80 countries since 1994. Our focus is on improving the safety of patient care through the provision of accreditation and certification services as well as through advisory and educational services aimed at helping organizations implement practical and sustainable solutions. In September 2007, JCI received accreditation by the International Society for Quality in Health Care (ISQua). Accreditation by ISQua provides assurance that the standards, training and processes used by JCI to survey the performance of health care organizations meet the highest international benchmarks for accreditation entities. In fact, the world’s first World Health Organization (WHO) Collaborating Centre, dedicated exclusively to patient safety solutions, is a joint partnership between the WHO, The Joint Commission, and JCI

I am also using a surgeon that no one here has heard of or talks about. His name is Horacio Guajardo and he's out of Monterrey, too. It's definitely not a bad thing that no one on these boards has used your doctor, it just means you'll have to do extra digging before making your decision. I searched his name and came up with one profile for someone who has used him. She is very positive about her experience there. If I were you, I'd contact her. http://www.obesityhelp.com/member/cristy/
I think it's okay that our surgeons don't advertise and that they don't have their coordinators crawling all over these boards. Just do your research, check out his credentials, if he says he's a member of an American organization, that's easy enough to check. Checking the JCI accreditation of the facility where he performs his surgeries is easy to do, too.

Whether being JCI accredited is a deal breaker for you or not is a personal thing. For me, it absolutely is. For others, it's not. It's our money, our health, our time, and our level of comfort.
Good luck in your decision making!
