any information about fmla leave from work

does anybody have any info on fmla or shortterm leave from work ...do i summit in advcance? who fills them out...for how long should i ask?  all the help i can get is useful

In our office we have an HR person to help us. I submitted the form for leave, the Doc fills out a form for HR, and they put it on the books. Was pretty easy to be honest. I work for a large company though, so that makes a difference. Do you have an HR person or a personnel person you can contact in your office?

I used fmla when i had rny in 2005.  I got 6weeks off.  I called my HR people a couple of weeks ahead and then recapped the day before. It is pretty easy.
