Which Insurance...

I know there is a page on here where you can post your insurance provider and their requirments. Most of those I'm finding are thru employers and we all know that employers decide what kind and how much coverage to provide based on how much money they can save.

What I'm looking for are companies that you can go to for insurance for 100% self-pay coverage. Does anyone know a resource for this? I need to find an insurance company to cover this and cannot quit my job and hope to get lucky with my next employers coverage.

I have basically always self-paid for my insurance, as I have pretty much only worked for mom and pop businesses, in a field that doesn't offer many benefits.  I have Blue Cross/Blue Shield of Michigan, and my policy pays for WLS 100% as long as you can meet their requirements -- at least one years worth of documented weight loss attempts by your PCP, and a psychological evaluation.  It paid for my recent surgery, which was billed as $46,000, but of course, the hospital accepted a lot less because they have a contract to do so.  I just have to pay a $250 a year deductible, and then everything else medical wise is covered, except no prescriptions (including things like wheelchairs and cpap machines), no office calls, and no "routine" like physicals (I NEVER understood the logic of this -- wouldn't it be cheaper in the long term to pay for preventative care than to treat the disease?) I pay about $275 a month for coverage -- is going to go up in the spring again, I hear.  But, I still think it's a bargain compared to what it would cost without it. I assume Utah also has a BCBS insurer.  The "Blues" are non-profit, and they really do, in my experience, try to make it easier for people to get the treatment they need.  I'm sure others will disagree with me on this, but they've always been good to me and my family, especially my sister, who has severe RA and who has had blue cross go above and beyond more than once to allow her to get surgeries which weren't available in Michigan.
