If your looking for sosmetic results...just having the panni removed (the panniculetomy..sp?) will not be enough for most people.  There are many issues that accompany going from SSMO to normal.....you might have a saggy butt.....you might have larger then normal breasts....better known as moobies...you might have saggy thighs, or sattlebags (butt wings) or bat wings.   Bottom line is you have to decide how much you want, how much you'd be will to spend, and meet with a plastic surgeon to discuss it.    I started at 492, at 230...i have major skin issues.  I've already been to a plastic surgeon and will need multiple surgeries to get my skin to look normal.  That will include a lower body lift, an upper body lift, thighs, and arms....and possibly a face lift (saggy neck area).  It's a little bit overwhelming in terms of cost.....there is only one surgeon on the east coast who will do most of those procedures at one time (dr. Hurwitz), but he costs about 50k ...and none of it is covered by insurance.   So i'll probably go the route of doing little at a time (FYI..male breast reduction can be covered by insurance in some cases).  Scott
