RI Healthmate coast to coast, ? cover surgery done in Boston, ma??

I just wanted to know if anyone has info on RI BCBS Healthmate coast to coast, and if they are picky on where you have surgery? It is a ppo plan if this helps.
Also if your BMI is 41, does BCBS usually approve the GBP?
