I know that with the gastric bypass you have to go through so much stuff and my insurance will not pay for that thank you so much.

I think the first thing you need to do is find out if your insurance covers WLS at all.  Some don't .  They don't care how your life or their bottom line would be improved they just don't cover it.

If this is the case then you can still try for an approval but you need to be prepared to fight it out tooth and nail so document EVERYTHING.  You may have to go through multiple appeals.  I have also heard of some people working through the HR with their employers but the employer had to give the Ins Co. permission to grant the surgery -   I have no idea how that works.

If they do cover WLS  then you should be able to call the INs. Co and request a list of requirements directly to meet their criteria.

I was self pay but my friend did hers through insurance.

She had to go on a six month diet to make sure she was going to stick to the diet and she was required to go to support group meetings

she had to have a  EKG,  blood work x 3  ,  Chest Xrays, Upper GI done , and had to go talk to a psychiatrist,

I had to do all them things also but I didn't have to do the 6 month diet thing or the support group. I did the support group meetings on my own to do learn about my doctor and how his patients really did with the band .

Hope this helps you a little bit . I'm sure somebody with more knowledge can  help you out.

I had to send a letter from my PCP stating my weight and BMI and his opinion on surgery.  Then I had to submit 3 months of weigh-ins.  I had to have a psych evaluation and that was it.  The longest part was waiting for the surgery date, I had to wait 3 months because my surgeon was booked up.
