I need help!!!
I am hoping someone knows the answer to this question. I see two different physicians - one for when I am sick, the other for everything else. When I initially spoke to my insurance about the requirements for my insurance approval, she (bcbs) told me I needed 6 consecutive month PCP forms for my surgery to be approved - no mention of it having to be the same doc. I had my first four months completed by one physician and the other 2 completed by my other doc. Now, I am being told by the lady over the insurance at the lapband doc that it needs to be 6 months of forms by same doc! Anyone know or had the same experience? I would appreciate any advice as this was not mentioned to me initially (I didn't think it would matter) and now I may have to start completely over on my 6 month visits which will obviously delay my surgery!! Arghh!!

I would fight that if I were you. Ask to see where it says it HAS to be with the SAME doctor.

I agree with pumpkin I would appeal it if it doesn't say it has to be the same doctor

My insurance (aetna) has posted on their website the exact requirements for wls. I'd check your insurance website. If there's no mention of there having to be one doc I'd definitely fight it. I deal with insurance companies everyday for work. If they can weasel out of paying they will. They're counting on people not fighting them!!
