Sounds like you've done exceptionally well, even with dealing with major surgery! Congrats on keeping your regain to a minimum!
I got off track over the holidays, and got back in gear by going low-carb. Here's a link to a thread where I responded to a poster's request for information on foods to eat. Scroll down, and I explained my current low-carb eating plan:

As for exercise, if you haven't been able to do much for a while, how about just walking? It's the perfect time of the year to get outside, and enjoy Nature's renewal along with your own! I like to listen to my iPod, and one of my favorite things to help me keep a good pace is using Podrunner, which is a Podcast available at the iTunes store. You can select the program based on the bpm (beats per minute). Most are geared towards running, but the ones in the 130-131 bpm range are excellent for power walking.
