Restaurant Card?

Just wondering if anyone knows where I can find the card to bring to restaurants that says you can only eat a child's portion...or whatever it says on it.

Mine was in my my Allergan Lap-Band information packet I got at the time of my surgery.

i got mine from my surgeons office...call them they should have them

I got mine when I got my first fill. I haven't had the guts to use it yet. Anyone else experience problems with restuarants not excepting cards?

Print it on heavy paper, like business card stock.

I have had a lot of LUCK with them accepting the card, or allowing me to order differently even without showing a card. Here in Austin, Red Robin will do whatever they need to accommodate us because their owner has had gastric bypass, Jason's Deli allows me to order the child size salad bar, and Catfish Parlour allows me to order catfish a la carte (99cents/piece) without having to pay for all the extras that I won't eat. Most other places I just eat off my husband's plate. I won't hesitate to use it at all, because otherwise food goes to waste. Once they see me carry a to-go box on an order of just three pieces of fish, they're a believer!
