Call them and see if they cover WLS.  Many states are simply running out of money, there aren't enough dollars so they are cutting benefits.  I believe AZ is one state that no longer does WLS under their various welfare plans.  :o(

Most finance companies require a credit score of 650, that's pretty darn good credit.

Have you considered finding a less greedy surgeon?  ;)  I do believe it is greed when these surgeons do this, they feel the thousands they make from the actual surgery isn't enough so they found a legal way to charge even more.

Not all surgeons charge program fees, I have seen them range from $0 to $7K.

Many surgeons believe the thousands they get for the surgery isn't enough, it is greed.  Some surgeons charge as much as $7,000 for a program fee.

If you can't find a surgeon that does not charge a program fee then the fee will have to be paid.  It's completely legal for them to do this.

This completely depends on the reason for the denial.  Sometimes they have a rule of 1 WLS per lifetime, sometimes they just need additional info, sometimes you don't meet the requirements.  What was the reason for denial?  Appeals depend on this information.

Yes the surgeon is just trying to get some extra bucks and calling it a program fee. I would check around even if you have to drive a bit to find a surgeon with no program fee or a more reasonable one. Gl

I'm so stressed out! My first option to fund my $4000 program fee fell thru, so I'm going to apply to one of these companies online. My credit is not great, and I see so many horror stories from these companies!!!!

Can you recommend a reliable source? I'm freaking out here!!!

If you meet the plan's criteria, you will get approved. IMO, UHC did little to try to connect any dots, so make sure your submission is water tight and you should be good to go!

I recommend you get a copy of your plan's guidelines if you haven't already. A call to customer support can tell you if your provider/hospital are in network.
