I was wondering if anyone has been approved with this insurance and what you needed to show as far as a diet to get the approval. The surgeon I picked wasn't able to get a clear answer from my insurance company on this and was reluctant to help me get the ball rolling on my surgery.

You need to find out from the company what their guidelines for approval of bariatric surgery are. They are supposed to provide you with this information upon request. What they like to do is wait until you are way into the process and then pull these out of their hat.
