I AM NEED OF SERIOUS HELP

I GOT A CALL TODAY FROM MY DOCTORS OFFICE SHE STATES THAT THE INSURANCE NEEDS MY DIET HISTORY (NO BIG DEAL I CAN DO THAT HAVE THAT FROM 3 DIFFERENT DOCTORS) THE ONLY PROBLEM IS THAT I HAVE TO HAVE A PSYCH EVAL. DONE OK AGAIN NO BIGGY BUT THE DOCTORS THAT TAKE MY INSURANCE WANT TO TAKE ME ON AS A NEW PATIENT AND FIND OUT WHATS WRONG WITH ME. I KNOW WHATS WRONG WITH ME I AM FAT AND HAVE BEEN FOR SOMETIME NOW. CAN ANYONE FROM SOUTHER INDIANA OR LOUISVILLE AREA HELP ME GIVE ME SOME NAMES OR PHONE NUMBERS

no i have a surgeon i have to have a psych eval done for insurance. i have sagamore insurance
