A doctor's letter...
Last week I paid a visit to the doctor that I used to see when I was married. During 2001 I was on a one year supervised diet and BCBS had recommended that I visit the doctor to get him to provide more supporting documentation because the medical records did not seem to provide enough.
This letter brought humor to my evening. I know that I will be approved for this surgery and it will ONLY be by the Grace of God.
I thought I would share this letter with you all....
"To whom it may concern:
I am writing this letter at the request of Beth (last name), a patient I have not seen since February 22, 2002. She asked that I detail my attempts to assist her lose weight between January 2001 to January 2002. During this time, I prescribed phentermine intermittently along with suggesting appropriate diet and exercise. She initially weighted 304 lbs and lost down to 227 lbs. I understand that my records regarding her treatment have been made available for review. I hope this information has been useful.

Doctors signature/End of letter
Also I just received my EOB for this visit which cost $73. I think when this is over, I could write a book.
