BCBS FEDERAL FOR PLASTIC SURGURY
I have BCBS - Empire, out of NY. My experience is that all the BCBS's have different utilization review guidelines for RECONSTRUCTIVE surgery.
For instance - Empire, BCBS requires 500 grams of extra tissue to approve a breast lift (mastopexy.) BCBS of Georgia only requires 150 grams.
When your surgeon sends a request for preapproval, you will find out how difficult Federal is. If you are initially denied, the surgeon can call for a peer to peer review with a staff physician. At that time, they will find out exactly what your BCBS requires.
Hope it's a slam dunk for you, and that you get your new body shortly. You don't mention what procedure you want, but building a strong case of documentation BEFORE you get to the surgical consultation stage is the way to go.
