ERISA question

I have a question. Has anyone been able to get WLS approved under an ERISA plan that WLS is excluded? Her insurance company is CIGNA btw. I know this sounds like an impossible dream... I have a friend who actually has a precancerous condition that can't be cured but won't get any worse by having the surgery. Its a disease in which her acid reflux causes the acids in her stomach to produce precancerous cells in her esophagus. Anyway, if anyone knows what she has to do to get this surgery approved, I would appreciate it. She already beat breast cancer, I really don't want her to go through it again.... Dawn
