advice please
I submitted a 12 month diet that I did in 2001 to BCBS of IL (which is one of their requirements) and they denied stating that it was not enough documentation - although the doctor did notate the weight that I lost and that I was exercising (walking) it was still not enough. I can appeal and my health insurance is an ERISA plan.
I went for a sleep study Sunday night and went back for the results today. I do have Mild-Moderate obstructive sleep apnea. I hope that these records may assist in the appeal that I send in to the insurance.
Any advice on what I can do to succeed in my appeal?

I have BC/BS of IL as well. I sent in about 8 years worth of information with numerous entries from my doctor recommending surgery, stating I was on Weigh****chers and lost xxx and they recommend I continue, that I saw a nutritionist etc. I even provided about 4 months worth of documented history showing I took Xenical w/ weigh-in information (It was about 5 year old). I was also denied for lack of information showing a "medically supervised diet within the last 24 months". They want it to be supervised by an MD etc. So it may be the "within the last 24 months" portion that they are hung up on or something.
I am hoping to hire Gary for the appeal since I was told that with BC/BS of IL you only get one..so I don't want to blow it. I am just scraping together $$ and getting paper work together.
