United Health Care written exclusion policy

Hi this link was sent to me by another member. I am looking for anyone with United Health Care insurance with a written exclusion in their policy that has been approved for WLS. My daughter has letters of medical necessity from our Doctors. Katie is just 17 and I have been trying to get an approval for 18 months. Tomorrow May 4 we have a second level appeal hearing.
