Help:) Anyone have a copy of an appeal letter?

Hi Amos family, i have bc/bs oa pa. and have been denied 2 times for abdominplasty/pannicelectomy.........they know want a personal letter and progress notes from the dr. does anyone have a appeal letter for ps i can look at to get ideas on how to write a good one?
