Humana

Hello Eveyone.. I have a quick question ..I am trying to find out my insurance requirments for wls.I have Humana someone posted the link to look up the requirements but I can not find it.So I called to get them to mail me a copy.I asked for a certificate of coverage but they say they only mail that out if you are cancling you insurance.So maybe I am asking for the wrong item..Could someone please point me in the right direction.The page I went to before had all the requiremnts you had to meet for them to approve the surgery and other types of procedures as well...Thanks Teesha
