IL Medicaid Approval Requirements

Can anyone tell me what requirements you need to meet in order to get Illinois Medicaid to approve you? I also have Health Alliance PPO and they have already approved me, but I need Medicaid to take up the deductibles and the copays for me. If you have any suggestions, please let me know - I am hoping to get my surgery date and have it within the next two months. Cindy A.
