Supervised diet requirements

Hi! Please help me compile information to challenge a diet requirement. I am fighting to get the surgery done and the only thing the company has to deny on is a requirement for 3 months diet in the past 6 months. It is a requirement that I cannot meet because I started serioously pursuing WLS in the end of September and the policy expired December 31 and the new plan excludes WLS. I am trying to get the coverage extended because the insurance company dragged its feet to run out the coverage. I meet every requirement except the 3 month diet. I am compiling information for the Insurance Department to try to show that this is an arbitrary requirement not based on any true medical literature. I have a lot of studies, but what I would like to add is a list of other insurance companies and what their requirements are to show that there is no consensus on this issue. If you can help, please email with the name of your company and the number of months (years) that they are requiring you to have supervised diet. Thanks to everyone who takes the time.

My insurance company (BCBS of IL) at the time demanded 12 months of supervised dieting under my PCP. Donna
