Advice for my "failed 12 month diet"

My new insurance policy states that in order to have wls I have to have a "documented failure of 12 continuous months of compliance with medically supervised non-surgical methods of weight reduction. Supervision must occur under a MD, DO or NP w/in the previous 24 months prior to the request. It should consist of: 1. Nutritional Therapy including a very low calorie diet unless contraindicated 2. Behavior modification or behavioral health interventions 3. Supervised increase in activity 4. Pharmacologic therapy unless contraindicated 5. Maintenance support to continue to encourage nutrition choices Do any of you have examples of your physician notes. Do we have to show our weight? Isn't it funny how they want a FAILED DIET? Seems like they would want a successful diet to show that we are committed to stick with it after we get the tool. Beth

sounded like my insurance company. I saw my PCP every month and had her make notes on what I was trying to do in order to lose the weight. Walking is all I could do because of my knees hurting so badly. Tried the diet pills, failed, tried the cutting back on food, failed and even did Atkins again. I ended up gaining weight and it was due to stress from the insurance company and my health. GO FIGURE!! Donna
