Everyone get ready.... HERE I GO!!!! I am now closer than I've ever been to getting gastric bypass. I am in the process of electing my insurance for this year. Out of respect for my new employer (which is absolutely marvelous), I had planned on waiting about 6 months before I had the surgery - regardless of insurance requirements. Here are my new requirements in order to have wls: -Letter of Medical Necessity including a proposed CPT code, height & weight, BMI, and a list of previous diets & programs attempted. -My health history or physical with a listing of comorbidities -Office notes for past year -detailed notes of 12 month supervised weight loss **** Here is my question... what is exactly considered a 12 month supervised weight loss? Did it have to happen in the past 12 months? I need as many details that you can give me. I am so excited that this is finally here, regardless of how long I have to wait.

My 12 months of dieting could not have any breaks in them. I had to go and see my PCP every single month like clock work. She wrote down my weight and what I was trying to do to lose the weight and what I did as a workout. I also had all my Doctors write letters on my behalf stating why they felt that WLS would benfit me. If your not sure then call your insurance company and ask. Good luck Donna
