How long appeal?

I haven't been denied yet, but my physician's office told me the Anthem Ohio always denies for the lapband (I have no exclusions for wls). I plan on getting an attorney as soon as the denial arrives. So how long does the appeal system take after you contact the attorney? I realize that it's different each time, but there should be an average. Are some attorneys backlogged more then others or is that not where the time is lost? I know, "patience...Grasshopper".
