United Healthcare Options PPO

Has any used United Healthcare Options PPO and receive approval. Everytime I call to find out what pre-authorization criteria is they change criteria. My Doctor will be sending letter to insurance this week but, I am afraid to get hopes up.

I have UHC Options PPO and i have an exclusion so they won't pay for the dam surgery. Just check and make sure you don't have an exclusion. Also, UHC is not covering WLS as of Jan 1, 2005. Good Luck!
