I need advise, I just spoke with the nurse on the med management team(she is new only been there 2 weeks). She said I needed a psych evaluation and 6 weeks med. diet history. I was advised early by nurse I did not need a psych eval and only needed 6 months diet history - weigh****chers, jenny craig etc. I have this. she ask did i want her to submit it as is or put it on hold until I get six months history. If she puts it in it could get approved, if not I have to the six months with my dr. and appeal- what should I do? Lori

Here's the problem: If it's not in writing, it never happened. That's why I tell everyone not to bother with calling their insurance company. Read your Summary Plan Description booklet, have your surgeon submit for pre-approval and see what happens. If you are denied, the denial letter must outline exactly what you need to do to "perfect" your claim. Take it one step at a time. Don't get all worked up on "what ifs."
