Unicare PPO Classic Illinois

I'm going to have Lap Band...I have searched all posts and cannot find anyone with experience with this company. Please help if you can. I used to be mad that my company quit offering BCBS of Illinois, but from what they said at the seminar, and what i've read here, I'm glad now! lol

Bad news. Unicare is owned by Wellpoint / Anthem, and they own BCBS in several states (but not Illinois.) They have a BCBS mentality when it comes to claims. From what I have heard, WLS is not a covered benefit under Unicare. (Their employees call them UN-care.) Check your Summary Plan Description to make sure WLS isn't an exclusion in your plan.

I called the Surgeon's facility today and got ALL CPT codes from all of the required tests to the psych consult and consult to surgery. Called Unicare today, and In Network is paid 90%, max out of pocket is $500. I've already eaten up my deductible for this plan year (March to March) My surgeon and facility of choice are in network, thank goodness. They do require a psych consult, which they pay for, and preauthorization. I'm going for my consult on Jan 7th, so we'll see once they submit paperwork.
