the summary plan fund booklet that I received from teamsters 710 states thisEffective October 1, 2004 the funds new preferred provider organization is bc/bs of illinois. Your local 710 health and welfare fund will continue to administer your health benefits as well as process your claims. Bc/bs of illinois will be your PPO network. When you use the bc/bs network the doctor and hospital generally will file its claim with bc/bs. Bc/bs will then transmit the claim electronically to our office. In order to obtain the maximum benefits provided under your plan you must choose a bc/bs network provider. So do they go by the Teamsters 710 summary plan description, or do they follow bc/bs of illinois plan description? I have the plan description for teamsters. And there is NO information on the gastric bypass???? thanks
