HELP!!!!!!!!!!!!!!

I have IBEW (International Brotherhood of Electrical Workers) which is Southwestern Health and Benefit Fund. Anyone know anything about this insurance plan or it's coverages on lap band surgery?!?!?!? Any answers would be of major help. Thanks!!
